It is important that we interview you to ensure that the results of this important study are valid. This is your chance to be heard and we are very interested in your opinions and experiences. |

I: | We generate random numbers to ensure that all Ohioans with a telephone have an opportunity to participate in this study. Your answers to these questions will be completely anonymous and confidential. |

I: | I understand your frustration. The 'do not call lists' apply only to telemarketers and agencies that are trying to sell something. We are not selling anything. We are conducting a scientific research study on behalf of the Ohio State University and we are only interested in your opinions and experiences. |

I'm calling from The Ohio State University and we're conducting a scientific survey designed to help research scientists understand important information about the efficacy of self administered Internet surveys compared with interviewer assisted telephone surveys. |

I: | The information which comes from this interview will provide a great service in regard to research efforts of The Ohio State University. We are very interested in your opinions and experiences. If a household member is selected to participate, they may receive up to $30 for their participation in a one-hour survey that will take place several months from now. |

I: | I understand that you are busy and your time is very valuable to us. Our screening interview is very short and will take about 10 minutes and only includes basic household information. We can start the interview now and finish later if necessary or schedule a time to call back. |

R: | WHY ARE YOU DOING THIS RESEARCH / STUDY? |
I: | The purpose of this study is to compare survey responses and interview experiences between self-admininstered Internet surveys and identical surveys given by a telephone interviewer. This will help us to understand what kind of differences might arise if respondents are allowed to pick between these two modes of interview in the same study. |
R: | HOW LONG HAVE YOU BEEN DOING THIS RESEARCH? / UNTIL WHEN WILL YOU DO THIS? |
I: | We started the survey right after Thanksgiving and will continue for several months, until we have enough participants to make scientific conclusions about how we can collect accurate data efficiently. |
R: | HOW LONG WILL THIS TAKE? |
I: | The interview will take less than 15 minutes and I will go through it just as quickly as I can. If you don't have time to answer all of the questions, I can always call you back to finish it at a later time. |
R: | WHAT ARE THE QUESTIONS LIKE? |
I: | The questions are basic demographic data like the number of people in your household, whether you have children, and whether you are currently working or going to school. You can refuse any question that you feel uncomfortable answering. All of your answers will be kept completely anonymous and confidential. |

I: | We need opinions from all types of people. It's okay that some questions won't apply to you - it just means that the interview won't take long at all to complete. |

I: | We appreciate your interest in participating in our survey but we have chosen to do this part of the survey over the phone rather than sending paper questionnaires through the mail or set them up in a website because we believe we can obtain better answers from our respondents in this way. Also, we can quickly address any questions or concerns that you may have regarding the questions. When we do the follow-up study in a few months, about half of the respondents will be asked to complete the interview over the Internet. You might have the opportunity to be one of those respondents. |
R: | HOW DO YOU INTERPRET THE DATA YOU GATHER FROM THIS SURVEY? |
I: | Your data will be loaded into our secure computer server along with the data from all of the other participants. We will take your name off so that the small number of researchers who have access to the data won't know who you are. Then, we will look at the responses of all of the people who completed the study and determine whether the Internet is an efficient way to collect the kind of detailed data we need. |
