There are a few conventions used in the questionnaire to be aware of before viewing or downloading the document.
1. The question numbers appear in the top left corner.
2. Questions that are read to the respondents are written in lower case. Statements intended only for the interviewer are in upper case.
3. The answer choices are listed below the question text. The number corresponds to the code stored in the data.
4. The branching can be traced by following the question listed before the "Go To:" statement. If there are no branches, there is a single "Go To" statement that is the default next question.
5. Statements surrounded by slash, star, slash which looks like: /* */ (for ex. /* Is the respondent male or female? */) are used to explain the internal check functions used in the survey. These questions are not seen by the interviewer or the respondent.
6. Loops are often used in the survey to collect information. The start of a loop has a question name that ends with LOOP-BEGIN and the end of a loop is signified with LOOP-END in the question name.
